Nursing Home Care

Nursing Home Care is the care for chronically ill persons; they have acquired a long a long-term illness and may need palliative or revalidation care. For admittance to nursing home care the client is visited at home, in the hospital or comes to St. Martin's home to visit the physician and the nurse manager for an intake. Nursing home care is regulated in the AVBZ law (algemene wet bijzondere ziektekosten voorzieningen or general legislation for persons with chronic illnesses). This requires scoring of the clients ADL (activities of daily living) possibilities.

Care Planning

The physician and the nurses based on the diagnosis and ADL score compile a care plan that is to be evaluated regularly. The clients, family and/or care takers are involved in the process of formulating the plan, because it is based on the philosophy of clients centered care.

Client care organization.

When a client is admitted there are many changes in a clients life that the client has to get accustom to. There are also many questions asked and answers to give. The task of the personal nurse/care taker is to guide the client through the many changes and new influences and impressions. The diagnosis and the clients' whishes and expectations are known on the day of admittance. Time(s) and date(s) are planned to sit with the client and complete the care plan.
